ABOUT US DECLARE WELLNESS CENTER
is an independent functional medicine practice in Murrells Inlet, SC, specializing in bioidentical hormone replacement therapy (BHRT), hormone optimization, and membership-based wellness care. Founded by Dr. Craig Cohen DC, FNP and Amanda Cohen, Declare was built around a simple belief: patients deserve a healthcare experience that is personal, proactive, and focused on addressing the root cause- not just managing symptoms.

ABOUT THIS POSITION
As Declare Wellness Center continues to grow, we are expanding our clinical team and seeking a Functional Medicine Provider (NP, PA, MD, or DO) to take full ownership of patient care alongside our founder. This is a meaningful, high-impact role at the center of everything we do- managing patient consultations, hormone and lab oversight, and individualized care planning across our active wellness programs and membership patient panel.
KEY RESPONSIBILITIES
Patient Care & Clinical Management- Conduct initial consultations, review-of-findings appointments, and follow-up visits for active wellness program members and VIP hormone patients.
- Manage BHRT and hormone optimization care, including developing and adjusting individualized treatment plans.
- Collaborate with the health coach to align medical and lifestyle interventions and ensure timely patient follow-up.
- Facilitate patient workshops and educational sessions to increase engagement and strengthen clinical touchpoints between visits. Lab Management & Prescriptive Oversight
- Review and interpret weekly lab results, order follow-up testing, and adjust treatment plans accordingly.
- Prescribe and manage hormone therapies, peptides, and related treatments within Declare's collaborative physician arrangement.
- Maintain complete and accurate clinical documentation within required EMR timelines. Practice Operations & Team Collaboration
- Participate in daily morning huddles and weekly team training sessions.
- Attend regular clinical check-ins and case reviews with practice leadership.
- Utilize Declare's systems
- including EMR platforms, Google Drive, and shared templates — to maintain consistency and operational efficiency. Patient Engagement & Retention
- Track patient progress across active programs and VIP memberships, proactively identifying patients who may benefit from additional support or program transitions.
- Communicate with patients between scheduled visits to reinforce treatment adherence and continuity of care.
- Represent the Declare standard of care in every patient interaction
- engaged, knowledgeable, and genuinely invested in patient outcomes.
SKILLS & QUALIFICATIONS
Required Active NP, PA, MD, or DO license with prescriptive authority SC licensure or active eligibility to obtain prior to start date Minimum 2 years post-licensure clinical experience Genuine interest in and exposure to functional medicine and/or hormone therapy Strong patient communication and interpersonal skills Comfort with digital tools- EMR systems, lab ordering platforms, and Google Suite Preferred (Nice to Have) Experience with BHRT or hormone optimization Functional medicine certification (IFM, A4M, or equivalent) Experience with peptide therapy or genetics-based protocols Background in a cash-pay or membership-based practice model Personal engagement in health optimization, fitness, or biohacking 6.

COMPENSATION & BENEFITS
Salary:
Starting at $50- 60 per hour, based on experience
Bonus Structure:
