Job Description

Nurse Practitioner / Physician Assistant (Contract to Perm)

Key Details Location:

Orangeburg, SC (2 Skilled Nursing Facilities)

Schedule:

Full-Time | Flexible hours (8 AM - 8 PM) | No weekends or on-call required

Pay Rate:

$65.00 - $68.00 / hour

Duration:

3-6 months (open-ended; potential to convert to permanent) Key Responsibilities Conduct comprehensive physical and clinical assessments for acute/chronic medical and behavioral conditions. Manage accurate EHR documentation, including ICD-10 coding. Lead advanced care planning conversations and educate patients/caregivers. Collaborate with an interdisciplinary team to coordinate patient care plans.

Must-Have Qualifications Licensure & Certification:

Active SC state license, active DEA/prescriptive authority, and national board certification (AANP/ANCC for NP or NCCPA for PA).

Education:

Graduate of an accredited MSN, DNP, or PA program. Specialties Accepted (NP): FNP, ANP, GNP, or AGACNP.

Logistics:

Reliable transportation to travel between 2 local facilities; physical ability to lift up to 30 lbs and navigate stairs.

vaccination and clean pre-employment drug screen. Preferred Highlights 1+ years of clinical experience in long-term care or geriatrics. Background in managing complex geriatric care and end-of-life discussions.
