Job Description

Memphis Health Center is seeking motivated and dedicated Nurse Practitioners to provide diagnostic and therapeutic medical care to patients across a variety of primary care settings. The successful candidate will work in collaboration with physicians and other members of the healthcare team to deliver comprehensive, patient-centered care. Responsibilities include observation and evaluation of patients, conducting case histories and physical examinations, ordering and interpreting diagnostic tests, and developing treatment plans. Essential Duties & Responsibilities
  • Ensure confidentiality of patient information and provide appropriate communication with patients, families, and healthcare professionals.
  • Conduct comprehensive physical examinations, evaluate patient histories, and treat common episodic and chronic health conditions.
  • Emphasize health maintenance and disease prevention through patient education and counseling.
  • Develops and implements personalized, longitudinal care plans for patients with multiple chronic conditions to improve health outcomes and quality of life.
  • Proactively manages chronic disease progression through regular monitoring, medication reconciliation, and education to prevent acute exacerbations and avoidable hospitalizations.
  • Collaborate with supervising physicians and interdisciplinary care teams while maintaining responsibility for ongoing patient management.
  • Interview patients/families, construct medical histories, and assess patient behavior and interpretation of health needs.
  • Develop and implement short
  • and long-range treatment plans.
  • Prescribe medications and treatments while demonstrating a strong knowledge of pharmaceuticals.
  • Order, perform, and interpret laboratory and diagnostic tests.
  • Counsel patients and families on preventive care, medical conditions, mental health concerns, and treatment adherence.
  • Provide pre
  • and post-operative care and respond effectively to medical or surgical emergencies.
  • Maintain accurate and timely electronic medical documentation.
  • Communicate with collaborating physicians regarding patient status, particularly when changes in treatment are required.
  • Demonstrate cultural competency and awareness of age-specific and spiritual practices.
  • Participate in staff meetings, continuing education, and quality improvement activities.
  • Perform other duties as assigned to support MHC's mission of delivering high-quality primary health care services. Requirements
Education:
Master's or Doctorate degree in Nursing (MSN/DNP) from an accredited Nurse Practitioner program required.
    Licensure/Certification:
    Current and unrestricted Nurse Practitioner license in the State of Tennessee (Required).
    • Current DEA registration for prescribing authority (Required).
    • Current BLS certification (Required).
    PALS & ACLS
    certifications as applicable (Preferred).
      Experience:
      No experience required; 1-3 years of clinical nursing or nurse practitioner experience preferred.
      • Specialty Experience (Preferred): Geriatrics, Internal Medicine, Primary Care, or Urgent Care. Skills & Competencies
      • Strong clinical judgment and ability to work independently within scope of practice.
      • Excellent interpersonal and communication skills to effectively counsel and educate patients and families.
      • Ability to establish strong collaborative relationships with physicians, nurses, and support staff.
      • Proficiency in using electronic health records (EHRs) and clinical documentation.
      • High level of professional integrity, accountability, and adherence to confidentiality and compliance standards.
      • Demonstrated ability to manage multiple priorities in a fast-paced clinical environment.
      • Commitment to patient-centered care and cultural sensitivity. Compensation & Benefits

      From $102,500 per year
