09/03/2026 Job Description Job Description Accura Health is seeking an experienced PRN Field Nurse Practitioner to provide high-quality primary and post-acute care to patients in their homes throughout South, Central, and North Dallas. This position focuses on Transitional Care Management and house calls, with an emphasis on preventing avoidable hospitalizations and improving patient outcomes. The Nurse Practitioner will manage an assigned coverage area and work collaboratively with physicians, care teams, and internal clinical staff.
Qualifications:
Certified Nurse Practitioner with an active Texas license in good standing Minimum of two years of Nurse Practitioner experience Minimum of two years of primary care experience Current CPR certification Valid Texas driver's license, automobile insurance, and reliable transportation Legally authorized to work in the United States Willing and able to travel throughout the assigned Dallas coverage area Accura Health is an Equal Opportunity Employer.
