Job Description
Job Description We are seeking a board-certified Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP/GNP), or Physician Assistant (PA) to provide medical care to geriatric patients at an assisted living facility in Conroe, Texas . This position is ideal for a provider seeking flexible, part-time work with the opportunity to build ongoing patient relationships in a growing practice. The provider will conduct weekly rounds on established patients , providing follow-up care, medication management, and chronic disease management. The current patient panel includes approximately 20 patients and continues to grow , with new admissions occurring regularly. Responsibilities Perform routine follow-up visits for geriatric patients Manage chronic medical conditions including hypertension, diabetes, dementia, and other common geriatric conditions Perform medication reconciliation and adjustments when appropriate Coordinate care with supervising physician and facility staff Document patient encounters in the EMR in a timely and accurate manner Provide patient-centered care with attention to geriatric needs Compensation Paid per patient visit Current panel of approximately 20 patients , with continued growth expected Additional Opportunity For providers interested in expanding their scope, there may also be an opportunity to build a patient panel within our private practice located in The Woodlands/Spring area , focusing on primary care. Qualifications Active, unrestricted Texas Nurse Practitioner or Physician Assistant license Board certification as FNP, AGNP/GNP, or PA-C Active DEA license Minimum 1-2 years of primary care experience required Experience with geriatric or assisted living populations preferred Ability to work independently in a collaborative care environment Benefits Flexible scheduling Part-time commitment (approximately 1 day per week ) Independent contractor autonomy Opportunity to grow with an expanding patient panel To Apply Please submit your CV or resume for consideration.
