We are looking for a skilled and caring Nurse Practitioner to join our growing team on a part-time basis. Job Summary We are seeking a Nurse Practitioner (NP) to provide house call visits and wound care services to patients in their homes. This is a great opportunity for an NP looking for a flexible, part-time schedule (2-3 days per week) while making a meaningful impact on patient care in a home-based setting. Responsibilities Conduct in-home patient visits, including comprehensive assessments and follow-up care Perform wound care evaluation, treatment, and management (pressure ulcers, surgical wounds, diabetic wounds, etc.) Develop and implement individualized care plans Diagnose and manage acute and chronic conditions during home visits Order, perform, and interpret diagnostic tests as needed Prescribe medications and treatments within scope of practice Document visits accurately and timely in the EMR Coordinate care with physicians, caregivers, family members, and other healthcare providers Educate patients and families on disease management, wound care, and preventive health Maintain compliance with state and federal regulations, as well as practice policies Qualifications Active, unrestricted Nurse Practitioner license in TX Board certification (FNP, AGNP, or related certification) required Current DEA registration (or ability to obtain) Prior experience in wound care and/or home health/house call visits strongly preferred Valid driver's license and reliable transportation Strong clinical assessment, communication, and time-management skills Ability to work independently in a field-based role CPR/BLS certification
