Job Description
Job Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive primary care services, perform patient assessments, and collaborate closely with healthcare professionals to deliver exceptional patient-centered care. Your expertise will help improve health outcomes across diverse populations, including pediatric, adult, and geriatric patients. This position offers an exciting opportunity to utilize your clinical skills in a fast-paced environment dedicated to community health and wellness. Duties Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans Prescribe medications and treatments in accordance with state regulations and collaborative practice agreements Manage acute and chronic conditions across various specialties including internal medicine, geriatrics, pediatrics, behavioral health, and urgent care settings Perform diagnostic evaluations utilizing eClinicalWorks to document patient encounters accurately Provide health coaching, education, and counseling to promote lifestyle modifications and disease prevention strategies Collaborate seamlessly with physicians, specialists, nurses, therapists, and other healthcare professionals to ensure comprehensive patient care Participate in telehealth consultations Qualifications Valid state licensure as a Family Nurse Practitioner with current certification from an accredited program (ANCC or AANP) Prefer at least 1 year NP experience, but will consider new graduates Proficiency with EMR/EHR systems. Strong knowledge of medical terminology, physiology, pharmacology (including CPT coding), and diagnostic evaluation techniques Ability to work collaboratively within a multidisciplinary team while demonstrating excellent client communication skills Knowledge of HIPAA regulations and medical documentation standards ensures compliance in all patient interactions Join us in making a meaningful difference by providing top-tier family healthcare!
