Job Description
Job Overview We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our nephrology clinic team. The NP will provide comprehensive care to patients with acute and chronic kidney conditions, working collaboratively with physicians and healthcare staff to deliver high-quality, patient-centered care. This role involves managing chronic kidney disease (CKD), hypertension, dialysis patients, and related conditions in an outpatient setting. Responsibilities Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic evaluations Diagnose and manage kidney-related conditions such as CKD, end-stage renal disease (ESRD), electrolyte imbalances, and hypertension Develop and implement individualized treatment plans Order and interpret laboratory tests (e.g., BMP, creatinine, GFR) and imaging studies Manage dialysis patients, including coordination with dialysis centers Prescribe medications and monitor effectiveness and side effects Educate patients and families on disease management, lifestyle changes, and treatment adherence Collaborate with nephrologists and interdisciplinary teams Maintain accurate and timely documentation in the EMR system Skills Experience managing CKD, dialysis, or transplant patients Strong clinical judgment and diagnostic skills Excellent communication and patient education abilities Ability to work independently and as part of a team Familiarity with EMR systems
