09/02/2026 Job Description Job Description Job Overview We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will play a crucial role in providing comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. Responsibilities Conduct thorough patient assessments and physical examinations to determine health status. Develop and implement individualized care plans based on patient needs and preferences. Manage chronic conditions and provide education on disease prevention and health maintenance. Perform medical procedures such as IV insertion, suturing, and basic life support as required. Maintain accurate medical records and documentation in compliance with healthcare regulations. Collaborate with interdisciplinary teams to coordinate patient care, including case management for complex cases. Provide specialized care for populations including infants, elderly patients, and those requiring hospice palliative medicine. Stay updated on best practices in dementia care and other relevant areas of family medicine. Requirements Master's degree in Nursing with a specialization as a Family Nurse Practitioner. Current state licensure as a Nurse Practitioner with prescriptive authority. Proficient in performing physical examinations and patient assessments. Experience in managing medical records effectively. Knowledge of basic life support techniques and certifications. Familiarity with dementia care practices is preferred. Experience in infant care is advantageous. Skills in hospice palliative medicine are a plus. Ability to perform IV insertion and suture procedures is desirable. Strong communication skills and the ability to work collaboratively within a healthcare team.
