Job Description

Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. In this PRN role, you'll complete detailed histories, physical exams, and risk assessments, identifying care gaps and social determinants that impact health. You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve outcomes. Responsibilities Perform comprehensive in-home health assessments for diverse patient populations Document clinical findings, diagnoses, and care recommendations using EMR systems Collaborate with physicians, care teams, and health plan partners to optimize outcomes Educate patients and caregivers on disease management, medications, and preventive care Identify care gaps and social determinants of health impacting patient well-being Adhere to evidence-based guidelines, quality metrics, and regulatory requirements Maintain accurate, timely, and compliant clinical documentation Participate in quality improvement initiatives and case review activities Travel to patient homes and adapt care to varied home environments Uphold Matrix Medical Network's culture of innovation, collaboration, and patient focus Required Skills Comprehensive adult and geriatric assessment Chronic disease management (e.g., CHF, COPD, diabetes) In-home physical examination techniques Clinical documentation and EMR proficiency Diagnostic reasoning and differential diagnosis Care coordination and interdisciplinary collaboration Patient and caregiver education Knowledge of quality metrics and value-based care Time management and independent practice in the field Regulatory and compliance knowledge (HIPAA, CMS)
