Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments for health plan members. In this role, you will review medical histories, conduct physical exams, reconcile medications, and identify care gaps to support better outcomes and reduced costs. You'll collaborate with physicians, care managers, and payers while documenting thoroughly in EMR systems. Responsibilities Perform comprehensive in-home health assessments for diverse patient populations Review medical history, medications, and risk factors to support accurate care planning Develop and document care recommendations aligned with clinical guidelines and payer requirements Collaborate with physicians, care managers, and payers to close care gaps and improve outcomes Educate patients and families on disease management, preventive care, and available resources Use EMR systems to accurately chart visits and submit timely, compliant documentation Maintain quality and productivity standards in alignment with Matrix Medical Network protocols Participate in ongoing training, quality initiatives, and performance improvement projects Required Skills Comprehensive health assessment Primary care clinical decision-making Chronic disease management Electronic medical record (EMR) documentation Care planning and care gap closure Patient and family education Medication reconciliation Telehealth and remote care coordination Regulatory and payer guideline knowledge Time management and autonomous practice
