Job Description

Bon Secours Cardiology in coastal Hampton Roads, VA has a new opportunity for a Cardiology Nurse Practitioner or Physician Assistant!
OPPORTUNITY
We are seeking a highly motivated Nurse Practitioner (C-NP) or Physician Assistant (PA) to join our growing cardiology practice providing both inpatient and outpatient cardiovascular care. The Advanced Practice Provider will work collaboratively with cardiologists and the multidisciplinary care team to deliver high-quality, evidence-based cardiovascular care. This position offers the opportunity to participate in the evaluation, diagnosis, and management of patients with a broad range of cardiovascular conditions, supporting continuity of care between the hospital and clinic settings. The incoming APP will support an established cardiology patient population and assist with growing clinical demand within the practice. Expected outpatient volume averages approximately 14 patients per day, with inpatient census varying depending on hospital coverage. WORK ENVIRONMENT
Combination of hospital-based and outpatient clinic care Collaborative team of cardiologists and advanced practice providers Opportunities for professional development and subspecialty exposure 8:00am
  • 4:30pm-hours may vary in the hospital setting Weekend call once a month

ER EPIC EMR QUALIFICATIONS
Graduate of an accredited Critical Care Nurse Practitioner or Physician Assistant program Board certification as NP or PA Active Virginia license (or eligible for licensure) BLS and ACLS certification 1 TO 3 YEARS of Prior cardiology practice experience required

CLINICAL FOCUS AREAS
The APP will assist in the management of patients with conditions including but not limited to: Supervising and managing the Coumadin/anticoagulation clinic Managing prescription refills and medication adjustments Triage and management of patient phone calls and clinical questions Monitoring blood pressure management and cardiovascular risk factors Supervising and assisting with cardiac stress testing Caring for patient population with: Coronary artery disease Acute coronary syndromes Heart failure Cardiac arrhythmias Hypertension Hyperlipidemia Preventive cardiology and risk factor management
RESPONSIBILITIES
Perform comprehensive cardiovascular assessments, including history and physical examinations Evaluate and manage patients with acute and chronic cardiovascular conditions Participate in inpatient cardiology consults and rounding Provide outpatient follow-up care and cardiovascular disease management Monitor diagnostic testing including: cardiac imaging stress testing laboratory data Develop and implement evidence-based treatment plans Collaborate closely with cardiologists, hospitalists, nursing staff, and other specialists Educate patients and families on cardiovascular disease prevention and management Document encounters in the electronic medical record in a timely manner Participate in quality improvement initiatives and clinical program development
