Job Summary Our client is seeking a dedicated Nurse Practitioner/Physician Assistant to deliver comprehensive medical care to patients in their homes. This role involves conducting detailed health assessments, managing acute and complex medical conditions, and developing treatment plans while ensuring quality patient outcomes. Key Responsibilities Deliver comprehensive medical care to patients in their homes Conduct detailed health assessments, examinations, and evaluations Diagnose and manage acute and complex medical conditions Develop treatment plans and coordinate appropriate follow-up care Perform clinical procedures including wound care, suturing, and splinting Order, interpret, and utilize diagnostic testing to guide patient care decisions Educate patients and caregivers regarding treatment plans and ongoing health management Document patient encounters thoroughly and accurately within established systems Participate in care coordination efforts to support continuity of care Engage in discussions regarding goals of care and treatment planning Collaborate with interdisciplinary team members to ensure high-quality patient outcomes Adhere to clinical protocols, safety standards, and performance expectations Demonstrate compassion, professionalism, and cultural sensitivity in all patient interactions Qualifications Current, unrestricted Nurse Practitioner or Physician Assistant license in Virginia National board certification Graduate degree from an accredited Nurse Practitioner or Physician Assistant program Current BLS certification required ACLS certification preferred DEA registration and prescriptive authority Valid driver's license with a satisfactory driving record Ability to complete required driver training upon hire Experience Minimum of two years of experience as an Advanced Practice Provider Experience in settings such as Emergency Medicine, Urgent Care, Internal Medicine, Acute Care, Hospital Medicine, Family Practice, Cardiology, or Pulmonary Medicine Skills Strong clinical skills in acute and complex medical management Excellent communication and patient education skills Ability to work collaboratively in an interdisciplinary team Proficiency in using electronic health records for accurate documentation Benefits Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances).
