Job Description
Matrix Medical Network seeks a Nurse Practitioner to deliver in-home health assessments and ongoing clinical care for diverse patient populations. In this field-based role, you will conduct comprehensive evaluations, diagnose and manage acute and chronic conditions, prescribe medications, and develop personalized care plans that support quality, compliance, and cost-effective outcomes. Collaborating with an interdisciplinary team, you'll leverage evidence-based practice, strong documentation skills, and patient education to improve health outcomes. Responsibilities Conduct comprehensive in-home health assessments for diverse patient populations. Diagnose, treat, and manage acute and chronic conditions following evidence-based guidelines. Develop and update individualized care plans in collaboration with patients and caregivers. Prescribe and manage medications in accordance with state and federal regulations. Document visits accurately and timely in EHR systems to support quality metrics and compliance. Collaborate with interdisciplinary teams and payor partners to improve outcomes and reduce costs. Educate patients and families on disease management, prevention, and healthy lifestyle choices. Participate in quality improvement initiatives and adopt innovative care delivery models.
