Competitive + Full Benefits Join Our Team A prestigious nursing home facility is seeking an experienced Nurse Practitioner (NP) to provide primary care to residents in our Skilled Nursing Facility. This is an excellent opportunity for a provider who enjoys building long-term patient relationships in a supportive, collaborative environment with a consistent Monday-Friday schedule. What You'll Do
  • Provide on-site primary care to residents, serving as their primary provider
  • Perform assessments, diagnose, and treat acute and chronic conditions
  • Order and interpret labs and diagnostic tests
  • Prescribe medications and develop individualized treatment plans
  • Collaborate with nursing staff and the interdisciplinary care team
  • Conduct regular rounding and respond to changes in resident condition
  • Educate residents and families on treatment plans Qualifications
  • Active Vermont NP license
  • Minimum 6 months of Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience (Preferred)
  • Strong clinical judgment and communication skills
