Nurse Practitioner - Post-Acute Care (Cardiology, Nephrology) About the

We are seeking a qualified and compassionate Nurse Practitioner to provide specialized care in one of two focused areas: Cardiology, Nephrology . You will benefit from intensive specialty training to help you become a subject matter expert, supported by a collaborative team and supervising physicians. Job Duties & Functions include, but not limited to: Perform patient rounding and management at Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), Long-Term Acute Care Hospitals (LTACHs). Deliver comprehensive care to patients with chronic and post-acute cardiac, nephrology conditions , depending on your specialty assignment. Conduct thorough assessments, develop and implement care plans, monitor treatment progress, and adjust interventions as needed. Collaborate closely with supervising physicians, facility staff, and families to ensure coordinated, high-quality care. Serve as a key liaison and effective communicator with interdisciplinary teams and facility leadership. Complete documentation on your own laptop, allowing you to finish charting conveniently from home. Maintain compliance with clinical protocols, HIPAA regulations, facility regulations, and company standards. Entering notes into EMR system, PCC system and GoogleDrive spreadsheets within 24 hours. Annual participation in on call rotation.

Licensed Nurse Practitioner (NP) with experience in internal medicine, family practice, geriatric medicine, or preferably cardiology, nephrology, or pulmonary care. SNF, ALF, and/or LTACH experience is highly desired. Up to date ACLS, BLS, and Board Certification required. Passion for working in post-acute care environments and being a lifelong learner. Must have a personal laptop or device for EMR documentation.
Work Location:
In-person rounding at SNFs, ALFs, LTACHs, and similar facilities, with flexibility to complete documentation from your home.
