JOB SUMMARY
The Per Diem Nurse Practitioner or Physician provides inpatient secure withdrawal management, medical stabilization, and treatment services to adults with substance use disorders. This role includes scheduled clinical coverage as well as overnight and weekend on-call medical support, ensuring continuity of care, timely clinical decision-making, and appropriate escalation for patients admitted to ShoqweL Ya' LhaoLh-ew'xw, Lummi Nation's Secure Withdrawal Management and Stabilization (SWMS) program. Providers practice within Washington State scope of practice and program protocols under the clinical oversight of the Medical Director.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
include the following, and other related duties as assigned. 1. Provide inpatient withdrawal management and medical stabilization services to adults with substance use disorders. 2. Conduct comprehensive medical assessments including history and physical examinations for voluntary and involuntary admissions. 3. Diagnose and manage acute and chronic medical conditions encountered during withdrawal management and stabilization. 4. Prescribe, manage, and adjust medications used in withdrawal management including medications for opioid, alcohol, sedative-hypnotic, and stimulant withdrawal in accordance with program protocols and Medical Director oversight. 5. Monitor and manage medications for opioid use disorder (MOUD), including assessment of response, adverse effects, and stabilization planning. 6. Provide ongoing medical evaluation and treatment for patients admitted to the SWMS program. 7. Maintain clear, concise, and timely medical documentation in the electronic health record. 8. Collaborate with the Medical Director, nursing staff, behavioral health providers, and support personnel to ensure coordinated inpatient care. 9. Serve as a clinical resource to nursing staff regarding withdrawal progression, symptom management, medication protocols, and clinical decision-making. 10. Identify medical or psychiatric instability requiring escalation and initiate appropriate response including Rapid Response activation, Code Blue procedures, or EMS transfer when clinically indicated. 11. Participate in interdisciplinary staff meetings, case consultations, and care coordination as requested. 12. Provide clinical consultation and guidance to nursing staff regarding changes in patient status, medication management, and withdrawal complications. 13. Participate in the SWMS rotating on-call schedule to provide after-hours clinical support including telephone consultation, remote chart review, order entry, and guidance to nursing staff. 14. Respond to after-hours clinical concerns in a timely manner and determine when in-person evaluation, escalation, or hospital transfer is required. 15. Follow established chain-of-command procedures for medical deterioration, medication variances, patient safety concerns, or situations requiring Medical Director involvement.
MINIMUM QUALIFICATIONS
  • Current, unrestricted licensure in the State of Washington as an Advanced Registered Nurse Practitioner (ARNP) or Physician (MD or DO) required.
  • Clinical experience providing medical care in inpatient, emergency, hospital, or addiction medicine settings preferred.
  • Experience in addiction medicine or withdrawal management preferred.
  • Current Basic Life Support (BLS) certification required.
  • Advanced Cardiac Life Support (ACLS) certification preferred.
  • DEA registration for prescribing controlled medications required.
  • Must possess a valid Washington State Driver's license and meet eligibility requirements for tribal insurance.
  • Lummi/Native American/Veteran preference policy applies.

  • Knowledge of substance use disorders and evidence-based treatment approaches.
  • Ability to work in a cross-cultural environment and understand Native American cultural values.
  • Strong written, verbal, and interpersonal communication skills.
  • Ability to establish and maintain cooperative and supportive relationships with program participants and staff.
  • Ability to function effectively in a multidisciplinary healthcare environment.
  • Basic computer proficiency including electronic health records and Microsoft Office applications.
WORK ENVIROMENT & CONDITIONS
  • Work occurs within a secure withdrawal management setting.
  • May interact with clients experiencing acute withdrawal symptoms, emotional distress, or behavioral instability.
  • Flexible scheduling may be required including evenings, weekends, and on-call coverage.

REQUIREMENTS
  • Must pass pre-employment and random drug and alcohol testing as required by the LIBC Drug & Alcohol-Free Workplace Policy.
  • Position requires extensive criminal background check.
  • Must maintain strict confidentiality with highly sensitive medical and personnel information.
  • Must comply with Federal, State, Tribal, and local reporting requirements.
  • Must function according to standing orders and clinical protocols developed in consultation with the Medical Director.
