bout the Role As a HouseCalls Advanced Practice Clinician (NP or PA), you'll conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, preventive care role focused on identifying health risks, closing care gaps, educating patients, and collaborating with primary care providers to improve overall health outcomes. Key Responsibilities Conduct comprehensive in-home health assessments
Review medical history, medications, vital signs, and physical exams
Perform evidence-based screenings and point-of-care testing as appropriate
Identify and document diagnoses and care needs
Communicate assessment findings to primary care providers
Educate members on disease management, medications, and preventive care
Address social determinants of health and connect members with resources
Recognize urgent health concerns and facilitate appropriate follow-up
