Wound Care Provider We are seeking a dedicated Nurse Practitioner (NP), Physician Assistant (PA), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO) to provide wound care services across skilled nursing facilities in Montesano, Centralia, WA, and nearby areas . This is a mobile position that requires a provider who is comfortable working independently, traveling between facilities, and maintaining a flexible schedule based on facility needs. Employment:
Join the team as a W2 employee. Your Day-to-Day Travel to assigned skilled nursing facilities to conduct wound care rounds and provide patient care. Assess, diagnose, and treat chronic, surgical, and dermatologic wounds. Perform bedside wound care procedures, including debridement, as appropriate to your training and scope of practice. Provide dressing changes and assist with feeding tube management as needed. Collaborate closely with facility staff and interdisciplinary healthcare teams. Maintain accurate and timely documentation in the electronic medical record (EMR) system. Manage a patient caseload of more than 10-15 patients per day . Be prepared for early morning starts when required by facility schedules.
Who We're Looking For Preferred:
NP, DNP, PA, DO, or MD with extensive clinical and hands-on experience in surgical or sharp wound debridement , feeding tube management , wound vacs, and skin grafting .
Skilled Nursing Facility Experience:
Experience working in a skilled nursing facility setting is a plus.
Wound Care Experience:
Previous experience working with a wound care company is preferred.
Full Commitment:
This role requires a full professional commitment and is not suitable for providers who are only available for a few morning hours before leaving for another job.
Flexible Schedule:
Must have flexibility to accommodate facility schedules and rounding days.
Transportation:
Must have reliable and adequate transportation and be willing and able to travel between facilities.
Positive Personality:
Professional, approachable, and able to build strong rapport with facility staff and patients.
Self-Motivated:
Comfortable working independently, managing schedules, and communicating effectively with care teams.
Independent & Motivated:
Self-driven, an excellent communicator, and able to work autonomously.
Licensure:
Must have an active Washington NP, PA, MD, or DO license .
Availability:
Open to traveling to and visiting multiple skilled nursing facilities throughout Montesano, Centralia, WA, and nearby areas .
Compensation & Benefits Employment Type:
W2
Position Guaranteed Daily Rate:
$500 per day for the first 3 months of employment 35% production rate for NP/PA providers 40% production rate for MD providers
Additional Procedures:
Compensation for emergency G-tube services and skin grafting may be structured/paid differently.
Paid Training:
Compensation is included during the training and shadowing period, which is approximately 2.5-3 days .
