Providers work within an interdisciplinary care team to manage complex medical conditions, improve health outcomes, and help patients remain safely in their homes. Key Responsibilities Conduct comprehensive in-home assessments, including physical exams, medication reviews, and screenings Manage acute illnesses and chronic disease exacerbations Support members during short-term rehabilitation episodes Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams Conduct advanced illness and advance care planning discussions Educate patients and caregivers Travel extensively throughout Washington to provide home-based care

Required Qualifications NP:

ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

PA:
