Nurse Practitioner or Physician Assistant (full-time) NeoPath Health, Inc. - 5.0 Green Bay, WI Job Details Full-time 22 hours ago Qualifications BLS Certification DEA License Advanced practice provider Full Job Description NeoPath Health, Inc., a Minnesota based administrator of employer based medical services, is in search of a full-time family medicine mid-level practitioner to work at an employer-sponsored primary care clinic in the Green Bay area. Position Summary The provider will work in a family practice clinical setting. The provider will provide general and preventive care; wellness exams, treatment of acute and chronic illnesses, order lab tests and prescribe medications to children and adults. The ideal candidate will be able to build exceptional rapport with patients to optimize engagement and invest in the long-term wellness of employees and their family members. Principal Duties and Responsibilities Provide clinical care of patients including: Assessment Diagnosis/treatment of illness Ordering and interpreting medical diagnostics Chronic disease management and Wellness care Prescribing of medications, treatments, therapy, medical equipment, etc.
- Follow established safety and health measures
- Develop and Implement treatment plans
- Promote health and wellness
- Educate patients and families on self-care to ensure positive long-term patient outcomes
- Oversight of onsite CLIA waived lab
- Dispense prepackaged medications onsite
- Refer to outside providers as needed
- Present on various related health topics upon request Qualifications
- Unrestricted NP or PA-C license to practice in Wisconsin
- Wisconsin Prescriptive authority and DEA licensure
- BLS certification Full-time 36-40 hours per week.
