Job Description

JOB SUMMARY
The Nurse Practitioner - Primary Care is a licensed healthcare provider responsible for delivering high-quality medical and educational services to patients in accordance with established Nurse Practitioner protocols and within the scope of practice defined by the State of Wisconsin. This role functions under the supervision of a licensed physician and plays a key role in supporting comprehensive, patient-centered care. The Nurse Practitioner collaborates with multidisciplinary care teams to promote health, prevent disease, and manage chronic conditions, while ensuring compliance with clinical standards and organizational policies.
ESSENTIAL JOB FUNCTIONS
Obtains comprehensive medical histories and physical data to support accurate and complete patient records. Performs appropriate physical examinations and documents findings in accordance with clinical and regulatory standards. Orders and interprets laboratory and diagnostic tests based on patient needs and clinical findings. Synthesize clinical data to establish accurate diagnoses and develop individualized treatment plans. Provides patients and families with education regarding medications, treatment plans, and follow-up care instructions. Collaborates with interdisciplinary health team members to ensure coordinated and effective delivery of care. Initiates referrals to appropriate healthcare professionals and facilities as indicated by patient condition or treatment needs. Educate patients and families on strategies to improve, promote, and maintain optimal health, including disease prevention and self-care practices. Maintains accurate and timely documentation, including clinical records, medication lists, and follow-up care to support reimbursement and quality care delivery. Adheres to the organization's drug formulary and prescribing guidelines in compliance with internal policies and external agreements. Identifies barriers to care delivery or patient flow and contributes to the development of corrective action plans. Provides clinical supervision and support to staff, including direction in triage and urgent clinical situations. Demonstrates accountability for personal performance, resource use, and timely completion of responsibilities to support operational and financial goals. Follows established safety protocols and procedures to ensure a safe and compliant work environment. Prepares written prescription orders for medications, including controlled substances, in compliance with regulatory requirements. Participates in the development of patient education materials and identifies relevant community resources to support patient and family needs. Manages and triages patient telephone inquiries, providing appropriate clinical guidance and support. Actively engages in selected committee and quality improvement initiatives as assigned. Maintains strict adherence to scheduled work hours with regular and reliable attendance. Performs other duties as assigned.
EDUCATION AND EXPERIENCE
Master's degree in Nursing required. Preferred one to three years of nursing experience in a related specialty.
CERTIFICATIONS / LICENSES
Current State of Wisconsin Nurse License and Nurse Practitioner Licensure required. Current Drug Enforcement Administration (DEA) License required. Preferred BLS and Advanced Cardiac Life Support (ACLS) at time of hire, or within three-months of employment.
