Practice Environment:
Busy, collaborative outpatient clinic with a varied patient population Collaborate with physicians to expand individualized primary healthcare services. Diagnose and treat a variety of acute and chronic health conditions. Obtain comprehensive patient histories and perform assessments through observation, interviews, and physical examinations. Order, perform, and interpret appropriate diagnostic studies and laboratory testing. Provide preventive healthcare services, including: Health assessments and screenings Immunizations Preventive counseling Ongoing patient care and health maintenance Develop and implement individualized treatment plans for patients. Provide direct treatment and management of health conditions, including appropriate referrals to healthcare providers and community resources. Counsel and educate patients and their families/caregivers regarding: Preventive health and wellness Treatment options Disease management Available community resources Coordinate care with physicians, specialists, and other members of the healthcare team. Maintain accurate and timely documentation of patient care and nursing processes in accordance with specialty-specific standards and network nursing policies. Registration:
Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
Education:
Master's degree of Nursing required.
