University of Wisconsin Medical Foundation is hiring a Nurse Practitioner / Physician Assistant•Outpatient Allergy in Sun Prairie, WI. This is a full-time position.
Work Schedule:
100%, day shift working Monday•Friday from 8: 00 am•5: 00 pm. You will work at Eastpark Medical Center in Madison, WI.

The pay range listed reflects both titles. Relocation assistance may be available for qualified applicants Be part of something remarkable As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients. We are seeking a Nurse Practitioner or Physician Assistant to: Work collaboratively within a multidisciplinary health team to provide comprehensive care to allergy patients in an outpatient setting Treat a variety of conditions and allergic reactions, including: allergies, anaphylaxis, asthma, and immunodeficiency disorders At UW Health, you will have: An excellent benefits package, including health and dental insurance, paid time off, retirement plans, two-week paid parental leave and adoption assistance. Education:
Minimum•
If Nurse Practitioner:
Master's or Doctorate's degree from accredited
Nursing Program If Physician Assistant:
Bachelor's or Master's degree from accredited
PA Program Work Experience:
Preferred•2 years experience as an APP working in allergy, pulmonary, urgent care, ED or primary care
Licenses and Certifications:
Minimum•
If Nurse Practitioner:
Active RN and APNP license in State of Wisconsin AND Active board certification as a NP in the following: Adult; Adult/Gerontology Primary Care; Family.
If Physician Assistant:
Active PA license in State of Wisconsin AND Active PA board certification. All Nurse Practitioner and Physician Assistant applicants must have: Active DEA license or ability to apply for such license prior to or upon hire. CPR certification and advanced life support certifications if relevant to practice area (ACLS, PALS, NRP).
