Duties Conduct thorough patient assessments, including medical history review and physical examinations, to determine health status and develop personalized care plans Manage acute and chronic conditions across a broad spectrum of primary care needs, utilizing evidence-based practices Order, interpret, and discuss diagnostic tests such as laboratory results, imaging reports, and other relevant diagnostics to inform treatment decisions Provide preventive care services including immunizations, health screenings, counseling on lifestyle modifications, and health coaching Collaborate with an integrated care team — including behavioral health and care coordination — on patient care, follow-up, and referrals Maintain accurate, timely documentation of all clinical encounters in our EHR system Educate patients on disease management, medication adherence, and wellness strategies to empower informed health choices Requirements Current WV license as an MD, DO, NP, or PA (or eligibility to obtain) Active, unrestricted DEA registration Familiarity with EHR platforms for documentation and patient data management Ability to perform comprehensive physical examinations and patient assessments efficiently in fast-paced environments Excellent communication skills for patient education and interdisciplinary collaboration A genuine interest in community and rural health Ready to Work on Your Terms — and Make an Impact While You Do?
