In this vital role, you will provide direct patient care across various settings, including long-term care facilities, hospitals, assisted living communities, and home health environments. Your expertise in patient support, vital signs monitoring, and medical documentation will contribute to delivering high-quality, personalized care. This position offers an exciting opportunity to work with diverse populations, including seniors, individuals with disabilities, and those experiencing complex health conditions such as dementia or infectious diseases. Your energetic approach and commitment to excellence will help foster a safe and supportive environment for all patients. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts or transfer devices. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely communication of changes to the healthcare team. Support medication administration under supervision, ensuring proper documentation in electronic health record (EHR) systems like Epic or Cerner. Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments by implementing tailored activity plans and maintaining a safe environment. Assist with complex medical procedures such as catheter care, wound dressing changes, phlebotomy (blood draws), and aseptic techniques during sterile procedures. Maintain detailed medical records and documentation in compliance with HIPAA regulations and facility policies. Collaborate effectively with multidisciplinary teams in acute care settings such as ICU or Level I Trauma Centers, ensuring seamless patient care transitions. Requirements Valid CNA certification recognized by the state licensing board. Experience working in diverse healthcare environments including hospitals (ICU, emergency medicine), nursing homes, assisted living facilities, or home care settings. Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS). Familiarity with electronic health record systems such as Epic, Cerner, eClinicalWorks or similar platforms. Ability to perform physical tasks including lifting patients with Hoyer lifts and assisting with mobility needs. Experience caring for individuals with developmental disabilities or behavioral health conditions is preferred. Strong communication skills coupled with a compassionate attitude towards patients of all ages; experience working with children or geriatrics is advantageous.
