We are expanding our Denver Market and are seeking an experienced Physician Assistant or Nurse Practitioner who is passionate about relationship-based medicine, preventative care, and healthcare innovation. This is an opportunity to step away from high-volume, insurance-driven medicine and practice in a model focused on accessibility, quality, and meaningful patient relationships.

Smaller patient panels Longer appointment times Same-day and next-day access Direct communication with patients Collaborative physician support Minimal administrative burden A strong culture focused on patient experience and clinical excellence Position Summary The PA/NP will provide comprehensive outpatient primary care services to patients across the lifespan within Pinnacle's Direct Primary Care model. The ideal candidate has at least five years of family practice or primary care experience and thrives in a collaborative, fast-growing, patient-centered environment. This position is based in Centennial, Colorado, serving the South Denver region and supports both individual/family memberships and employer-sponsored healthcare partnerships. Responsibilities Provide comprehensive family medicine and primary care services Diagnose and treat acute and chronic medical conditions Deliver preventative and wellness-focused care Build long-term relationships with patients and employer groups Utilize telemedicine, secure messaging, and technology-enabled care Maintain timely and accurate medical documentation Collaborate with physicians and clinical staff to coordinate patient care Participate in community outreach, employer education, and DPC engagement efforts Support Pinnacle's mission of improving healthcare access, affordability, and experience Available for after-hours virtual urgent care needs of their enrolled population; cross-coverage available
Qualifications Required:
Licensed Physician Assistant or Nurse Practitioner in Colorado Minimum 5 years of family practice or primary care experience DEA registration and prescriptive authority eligibility Strong communication and relationship-building skills Experience managing patients across the lifespan Passion for preventative, relationship-based medicine
Preferred:
Experience in Direct Primary Care, concierge medicine, or value-based care Experience with employer-sponsored healthcare populations Comfort with virtual care and digital communication tools
Compensation & BenefitsCompensation Base Salary:
