In this vital role, you will provide personalized assistance to individuals in their homes or community settings, promoting independence, safety, and well-being. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia support. Your expertise will help create a comfortable, respectful environment that fosters healing and dignity. Responsibilities Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and document patient conditions through accurate recording of vital signs, medical records, and progress notes using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality. Observe patients closely for changes in condition, behavior, or safety concerns; communicate findings promptly to supervising healthcare professionals. Assist with specialized care needs including catheter care, hospice support, memory care for Alzheimer's or dementia patients, and working with individuals with developmental disabilities. Prepare nutritious meals and assist with feeding while considering dietary restrictions; perform light cleaning and laundry to maintain a safe and hygienic environment. Operate medical equipment such as Hoyer lifts and blood pressure monitors; assist with medical documentation and ensure compliance with safety standards. Requirements Valid CNA certification or Home Health Aide license required; additional certifications such as CPR, first aid, or medication administration are highly preferred. Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation. Experience working with individuals who have dementia, Alzheimer's disease, or other cognitive impairments; ability to provide compassionate memory care. Ability to perform patient observation accurately and respond swiftly to changing needs or emergencies. Excellent communication skills to collaborate effectively with clients, families, and healthcare teams. Physical stamina for lifting patients safely and performing caregiving duties throughout the shift; flexibility for live-in employment options is a plus.
