Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately Support medication administration following medical guidelines and HIPAA regulations to ensure safety and compliance Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Perform patient observation to identify changes in condition or behavior and communicate findings promptly to healthcare professionals and supervisors promptly Maintain detailed medical documentation per Agency protocols. Assist with household tasks such as meal preparation, cleaning, laundry, and maintaining a safe environment to promote overall well-being Ensure compliance with all safety protocols including CPR certification, and infection control standards Qualifications Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) required; prior experience in homes, nursing homes and/or assisted living facilities is highly preferred Demonstrated experience working with seniors, individuals with disabilities, or in long-term care environments Knowledge of medical terminology and medication administration procedures Ability to perform patient observation accurately and respond quickly to changing conditions Proficiency in using EMR/EHR systems for documentation; familiarity with home health software is a plus Strong communication skills combined with compassion and patience when caring for vulnerable populations Physical ability to assist with patient mobility tasks including lifting, transferring, and supporting patients during daily activities Join us in delivering exceptional home health care that empowers individuals to live healthier, happier lives.
