• Responsibilities:

• Provide skilled nursing assistance to patients in their homes, including but not limited to: + Assisting with personal care activities such as bathing, dressing, and grooming + Assisting with mobility and transfers + Monitoring vital signs and reporting changes to the healthcare team + Assisting with medication administration and other treatment plans
• Collaborate with the healthcare team to develop and implement individualized care plans
• Communicate effectively with patients, families, and healthcare providers
• Maintain accurate and detailed records of patient care and services provided
• Participate in ongoing training and professional development to stay current with industry standards and best practices

• Requirements:

• Current certification as a Certified Nursing Assistant (CNA) in the state of Florida
• High school diploma or equivalent required
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Ability to lift, transfer, and reposition patients safely and comfortably
• Reliable transportation and a valid Florida driver's license
