• PRN Job Overview We are seeking compassionate and dedicated Home Health Aides (HHA) and Certified Nursing Assistants (CNA) to provide exceptional patient care in various settings, including home health, assisted living, nursing homes, and hospice.

The ideal candidates will have a strong commitment to improving the quality of life for patients with diverse needs, including those with dementia, Alzheimer's, developmental disabilities, and other chronic conditions. This role offers an opportunity to work closely with patients, supporting their daily activities and health needs while maintaining a professional and caring environment. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance Monitor and observe patient conditions, documenting changes accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications and perform medication management following HIPAA guidelines and medical protocols Conduct vital signs measurement such as blood pressure, pulse, temperature, and respiration rates Provide specialized care for patients with dementia or Alzheimer's, including memory care techniques Assist with catheter care, wound care, and other complex medical procedures such as Hoyer lift transfers and hospice support Support patients with meal preparation and feeding, ensuring nutritional needs are met Perform light housekeeping tasks like laundry and cleaning to maintain a safe environment Document all patient interactions and medical records accurately and timely Follow infection control protocols and maintain patient confidentiality at all times Requirements Valid HHA or CNA certification required; CPR certification preferred Experience working in assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of medical terminology and medical documentation practices Experience with EMR/EHR systems for accurate record keeping Ability to work independently or as part of a team in a home or community setting Compassionate attitude with excellent communication skills for working with patients with disabilities or cognitive impairments such as dementia or Alzheimer's disease Physical ability to assist with patient mobility using equipment like Hoyer lifts; capable of lifting up to 50 pounds if necessary Working knowledge of HIPAA regulations to ensure patient privacy and confidentiality is maintained at all times Flexibility to work various shifts including live-in employment options; experience in long-term care preferred Ability to work with diverse populations including seniors, individuals with developmental disabilities, or those requiring hospice care This role provides an impactful career opportunity for individuals committed to delivering high-quality patient care in a supportive environment.
