Job Description

About Us Harmony Care Solutions is a Georgia Pediatric Program (GAPP) enrolled Medicaid home health agency providing in-home personal care support to medically fragile children under the age of 21. We serve families in the Douglas County area and surrounding communities. About the Role We are hiring compassionate, dependable CNAs and PSS Aides to assist medically fragile children with activities of daily living in their homes. All care is provided under the supervision of a licensed RN or LPN in accordance with each member's approved plan of care. Compensation $15.00 per hour Minimum Qualifications You must meet ONE of the following: Active Certified Nursing Assistant (CNA) certificate issued through the Georgia Department of Human Resources, Office of Regulatory Services; OR Pass status for a written competency exam approved by CHAP, JCAHO, NAHC, or ACHC; OR Completion of 40 hours of agency-provided training (at least 20 hours must be completed before working with members, with the remaining 20 hours completed within the first 6 months of employment) Additionally, all applicants must have: Ability to read, write, and follow verbal and written instructions Ability to complete legible written reports of caregiving activities Current CPR and Basic First Aid certification Satisfactory criminal background check including fingerprinting Reliable transportation to member's home Key Responsibilities Assist members with feeding, bathing, dressing, grooming, and personal hygiene Assist with mobility and ambulation Perform light housekeeping around the member's personal space Assist with meal preparation as directed Complete and sign the daily care task list and ADL checklist on each date of service Participate in bi-weekly supervisory visits conducted in the member's home by an RN or LPN Report any changes in the member's condition to the supervising nurse immediately Follow the individualized Caregiver Skills Checklist developed by the case manager RN Required Training Topics (completed within 90 days of hire) CPR and Basic First Aid Emergency procedures Infection control Personal hygiene and ADL assistance techniques Basic nutrition Home safety and maintenance Body systems and functions Chronic illness and disability awareness Confidentiality Communication and interpersonal skills Agency policies and procedures Meal preparation, laundry, and homemaking activities Important Program Notes All services must be prior-authorized — aides may not provide services outside the approved plan of care Supervisory visits by an RN or LPN are required every two weeks in the member's home Services may not be rendered in a hospital, school, or outside the state of
