Job Description

Job Summary We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA) to join our team. The ideal candidate will possess a strong background in patient care, with experience working in assisted living, nursing homes, or home health environments. This role involves providing essential support to clients by assisting with daily activities, monitoring health status, and ensuring their safety and comfort while adhering to all healthcare standards and regulations. Responsibilities Observe and monitor patients' conditions, documenting changes accurately in electronic health records (EHR) systems. Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts. Take vital signs regularly and report any abnormalities to healthcare professionals promptly. Support patients with medication administration following prescribed protocols while maintaining strict compliance with HIPAA regulations. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Assist with catheter care, hospice support, and other medical procedures as trained. Prepare meals according to dietary restrictions and assist with meal planning and preparation to promote nutritional health. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and general cleaning. Document all patient interactions thoroughly in medical records to ensure continuity of care. Collaborate effectively with healthcare teams to deliver comprehensive home or community-based care tailored to each client's needs. Requirements Valid CNA certification or PCA license required; prior experience in nursing homes or assisted living preferred. Demonstrated experience working with individuals who have developmental disabilities or dementia-related conditions. Knowledge of medical terminology, medication administration, and basic clinical procedures such as vital signs measurement. Proficiency in using EMR/EHR systems for documentation purposes. CPR certification is mandatory; additional training in hospice or Alzheimer's care is a plus. Ability to assist with ADLs using appropriate techniques and equipment like Hoyer lifts safely. Strong communication skills coupled with compassion and patience for working with diverse populations. Experience in home health or long-term care environments is highly desirable. Ability to perform light housekeeping duties including cleaning, laundry, and meal preparation while maintaining professionalism and confidentiality. $15.50 - $16.50 per hour
