CRNA Practice Profile Client :
Withheld Location/Worksite address : Morehead, KY Dates coverage is needed : ASAP - Ongoing Is this ongoing ?: Yes; Temp CRNA is exiting Schedule/Shift times : Mon-Fri 7am 3:30pm Call required ?: Oncall I night per week and one weekend per month Patient ages (children, adolescents, adults, geriatrics): Types of cases/Specialty mix : General Surgery, Bariatrics, Scopes, Urology, Ortho, OB/Gyn Head/Hearts?: No Support staff available (CRNA s, MA s, Scrub Techs): 2 docs and 3 CRNAs Type of charting (EMR, paper ): Meditech Credentialing timeframe: Two weeks
