In this vital role, you will provide essential direct patient care, support clinical staff, and ensure a safe, comfortable environment for individuals across various care settings. Your energy and enthusiasm will help promote healing, independence, and dignity for our patients. This position offers an exciting opportunity to make a meaningful difference in the lives of those you serve while working in a fast-paced, collaborative environment. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices. Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate documentation in electronic health records (EHR) systems like Epic or Cerner. Support clinical teams by preparing patients for procedures, assisting with medical documentation, and managing infection control practices including aseptic techniques. Provide specialized care for individuals with dementia or Alzheimer's disease in memory care or assisted living settings. Administer medications under supervision, ensuring compliance with HIPAA regulations and safety protocols. Assist with patient transfers, positioning, and mobility to prevent falls and promote comfort using appropriate equipment such as Hoyer lifts or wheelchairs. Collaborate with multidisciplinary teams in acute care environments like Level I Trauma Centers or ICU units to deliver high-quality patient service across medical-surgical, pediatric (PICU), or behavioral health units. Qualifications Valid CNA certification in good standing with the state licensing board. Prior experience working in long-term care facilities, hospitals (including ICU or emergency medicine), hospice care, or outpatient clinics is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or similar platforms. Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS). Experience with specialized care areas including dementia care, infectious disease management, post-acute care, or pediatrics is a plus. Ability to work effectively with diverse populations including individuals with developmental disabilities or disabilities requiring behavioral health support. Strong communication skills coupled with a compassionate approach to patient care and caregiving responsibilities. Join us to be part of a passionate team committed to delivering exceptional patient-centered care! We value energetic professionals eager to grow their skills while making a positive impact on the lives of others through dedicated caregiving and clinical excellence.
