  License/Certification (Companions Don't Need This) Annual Physical
• (Must Have)(Doctor stating you are well and have no limitations to work) PPD/Chest X-Ray
• (Must Have)(Chest X-Ray is a One-Time ONLY event once the initial chest-ray is completed, there is a annual TB Clearance Required) Criminal Background Check CPR
• (Must Have) First Aide(If Applicable) License/State ID
• (Must Have) Social Security Card
• (Must Have) Passport, Green Card, or Work Permit(If Applicable) Bring With You When You Come
