Job Description

Job Summary We are seeking a compassionate top notch Certified Nursing Assistants and experienced caregivers who can provide outstanding care for bed-bound patients and individuals requiring extensive assistance in the home care setting. Our ideal candidate demonstrates compassion, patience, professionalism, strong clinical skills, and a commitment to maintaining the highest standards of patient care. Responsibilities Assist clients with bathing, grooming, dressing, and personal hygiene Provide incontinent care and brief changes with dignity and respect Safely reposition and turn bed-bound patients to prevent skin breakdown Monitor and report changes in client condition Assist with transfers using proper body mechanics and equipment when needed Provide companionship and emotional support Assist with meal preparation and feeding assistance Perform light housekeeping related to client care Remind clients to take medications as directed by healthcare providers Maintain accurate documentation and care notes Follow individualized care plans and company policies Communicate effectively with families, nurses, and supervisors Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately within electronic health record (EHR) systems such as Epic or Cerner. documentation and adherence to HIPAA regulations and aseptic techniques. Assist with medical procedures including catheter care, wound care, and basic life support during emergencies. Provide specialized care for individuals with dementia or Alzheimer's disease, memory impairments, or behavioral health conditions with patience and understanding. Collaborate with multidisciplinary teams in acute care settings such as ICU or Level I Trauma Centers, ensuring seamless patient service across inpatient and outpatient environments. Maintain a clean and safe environment by following infection control protocols related to infectious disease care and hospital hygiene standards. Qualifications Current CNA certification preferred Minimum 1 year of caregiving experience Experience caring for bed-bound, hospice, dementia, or special needs clients preferred Reliable transportation and valid driver's license Ability to pass background screening CPR certification preferred Professional appearance and excellent communication skills Dependable, compassionate, and trustworthy Skills Proven experience working in diverse healthcare environments including hospitals (ICU, emergency medicine), nursing homes, assisted living facilities, or home & community care settings. Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation. Strong knowledge of physiology, anatomy, medical terminology, and patient care procedures relevant to geriatrics, pediatrics (including PICU), behavioral health, and post-acute care. Skilled in using medical equipment such as Hoyer lifts and performing catheter care or medication administration safely. Ability to work effectively with individuals with disabilities or developmental challenges while demonstrating compassion and professionalism. Experience in handling urgent situations requiring basic life support (BLS), aseptic techniques or trauma response in Level II trauma centers or similar high-acuity settings. Excellent communication skills for collaborating with healthcare teams and providing empathetic patient service across all age groups.
