This role involves regular travel between Jackson, Lenawee, and Hillsdale counties, providing Home-Based Care.

As a Certified Nurse Aide (CNA) in our Day Center and providing in-home care, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve in Jackson, Lenawee and Hillsdale counties. • Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting. Support Mealtime
• Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving
• Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks
• Measure and record vital signs, weight, and intake/output. Maintain Comfort & Care
• Keep rooms tidy, cozy, and well-stocked with essentials. Provide Responsive Support
• Answer call lights quickly and respond with kindness and compassion. Monitor with Watchful Eyes
• Report any changes in conditions to the leadership ASAP. Demonstrate Respect & Professionalism
• Maintain confidentiality while delivering top-notch care.

Participate in after-hour on-call rotations, including evenings, weekends, and holidays.

Rotation Schedule:

Be available for evening and weekend shifts as part of the rotation. If selected for the position, you will receive a full job description. Skills, Knowledge and Expertise Must be 18 years of age or older. A current and valid Michigan CNA Certification, required. CPR Certified, preferred. Six (6) months of clinical experience as a licensed CENA, preferred. Prior home care experience, preferred. The ability to read, write, understand, and follow directions. A heart for helping others and the physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed. Ability to lift a minimum of 35 pounds frequently is required.
