Job Description

Overview Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) specializing in Assisted Living and Memory Care! In this vital role, you will provide compassionate, high-quality nursing care to residents with diverse needs, including those living with Alzheimer's disease and other forms of dementia. Your expertise will support residents' health, safety, and well-being in a vibrant, community-focused environment. Duties Deliver comprehensive nursing assessments and develop individualized care plans tailored to residents' needs, including memory care and acute pain management. Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols. Monitor vital signs, perform physical examinations, and evaluate residents' health status regularly to identify changes requiring intervention. Assist with activities of daily living (ADLs), such as bathing, dressing, mobility support, and catheter care, ensuring dignity and comfort. Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner; ensure compliance with HIPAA regulations. Collaborate with interdisciplinary teams—including physicians, therapists, and case managers—to coordinate comprehensive resident care plans. Respond promptly to emergencies or urgent health concerns using triage skills; manage airway management and acute care procedures as needed. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in assisted living, long-term care, or memory/dementia care settings; hospital or ICU experience is highly valued. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation standards. Knowledge of infection control practices, sterile techniques, and aseptic procedures. Ability to work effectively with individuals with developmental disabilities or behavioral health needs. Strong assessment skills including vital signs measurement and patient evaluation; experience with tube feeding or ventilator management is a plus. Proficiency in EMR/EHR systems such as Athenahealth or eClinicalWorks; familiarity with medical imaging or laboratory procedures is advantageous.
