Job Description

Certified Home Health Aide (CHHA) - Hospice Care Part-Time | 7:00 AM - 12:00 PM | New Jersey Make a Difference Every Day Vitality Hospice & Palliative Care is seeking a compassionate and dependable Certified Home Health Aide (CHHA) to join our team. In this role, you will provide hands-on personal care and support to hospice patients in their homes, assisted living communities, skilled nursing facilities, and other care settings. You'll work closely with our interdisciplinary team to help ensure comfort, dignity, and quality of life for patients and their families. Responsibilities Provide personal care services including bathing, grooming, dressing, and mobility assistance Assist patients with activities of daily living according to the care plan Observe and report changes in patient condition to the supervising Registered Nurse Document patient care and services accurately and timely Promote patient comfort, safety, and emotional well-being Collaborate with nurses and other members of the hospice care team Follow all company policies, procedures, and regulatory requirements Travel to patient residences and care facilities as needed Qualifications Current New Jersey Certified Home Health Aide (CHHA) Certification required Valid driver's license and reliable transportation required Compassionate, patient-focused approach to care Strong communication and documentation skills Hospice, home health, or healthcare experience preferred Benefits Flexible schedule Paid time off Mileage reimbursement Travel reimbursement Flexible spending account (FSA) Supportive team environment Opportunity to make a meaningful impact in patients' lives
