In this vital role, you will provide high-quality, patient-centered care to individuals across various settings, including assisted living facilities, nursing homes, and private homes. Your dedication will help enhance the quality of life for seniors and individuals with disabilities by delivering personalized support, ensuring safety, and promoting independence. This position offers an engaging opportunity to make a meaningful difference every day while working in a dynamic and supportive environment. Responsibilities Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance. Monitor and observe patients' conditions closely, recording vital signs and reporting any changes to healthcare professionals promptly Support patients with specialized needs including dementia care, Alzheimer's care, memory care, hospice care and working with individuals with developmental disabilities or disabilities. Assist with meal preparation, feeding, laundry, cleaning, and maintaining a clean and safe environment in compliance with HIPAA regulations. Document patient interactions accurately while respecting privacy standards; ensure all records are up-to-date and compliant with medical records management protocols. Collaborate with healthcare teams to coordinate home & community care services that promote patient well-being beyond clinical settings. Skills Strong knowledge of medical terminology and patient care procedures. Experience working in assisted living facilities, nursing homes, or home health environments Ability to perform patient observation accurately and recognize signs requiring immediate attention. Compassionate caregiver experienced in dementia care, Alzheimer's care, hospice support, and working with individuals with disabilities. CPR certification or willingness to obtain certification; familiarity with hospice care protocols preferred. Excellent communication skills to interact effectively with patients, families, and healthcare teams. Ability to work independently or as part of a team in fast-paced environments; flexibility for live-in employment options is a plus.
