Job Description

RN - Team Leader Ensures quality and level of care for patients are up to established standards. Analyze acute care documentation and verifies that it reflects supported diagnosis and plan of care and meets reimbursement and regulatory standard. Primarily office setting utilizing basic office equipment and supplies. Involves problem solving, analytic skills and effective use of written and verbal communication skills with agency field, office and administrative personnel. Occasional visits to patient's homes within a 60 mile radius in all kinds of weather and occasional travel to conferences and meetings related to scope of practice. Must be able to access a wide variety of home structures. Accessibility to patient records on both paper and electronic format, state and federal regulatory data, reimbursement criteria, agency policy and procedure guidelines, system and patient reports, documentation process flow charts and manuals, coding texts and guideline manuals. Graduate of an approved School of Registered Nursing Demonstrates excellent knowledge through practice of Agency policy, procedure, and process. Demonstrates a basic understanding and working knowledge of coding concepts and federal and state guidelines as they relate to home care. Basic keyboarding skills to access data via lap top or personal computer terminals. Strong oral and written communication and interpretive skills. Minimum of 2-3 years clinical experience in acute care setting with a minimum of 1 year in primary case management experience in home care. Current State of Ohio Registered Nurse license Home Care Coding/ Specialist - Diagnosis Certification current or required within first year of position. COS-C Certification desirable.
