Job Description

Overview Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) here at Willow Creek Post Acute, where your compassionate care and attention to detail will make a meaningful difference in patients' lives. In this vital role, you will provide essential support to nursing staff and deliver high-quality, patient-centered care across various healthcare settings. We are currently hiring for all shifts (12 hour shifts) including PRN roles. Your energy, empathy, and commitment to excellence will help foster a safe, comfortable, and healing environment for individuals of all ages and needs. Duties Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote independence and comfort Take and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates Support medical procedures by preparing patients and assisting with tasks like catheter care, wound care, or basic life support during emergencies Document patient information precisely using electronic health record (EHR) systems like Epic or Cerner to ensure seamless communication among healthcare providers Monitor patients' conditions closely, report changes promptly, and collaborate with nurses on care plans for individuals with complex needs such as Alzheimer's or dementia care Help with medication administration under supervision, ensuring adherence to HIPAA regulations and safety protocols Use specialized equipment such as Hoyer lifts or assistive devices to safely transfer or reposition patients in compliance with aseptic technique standards Requirements Valid certification as a Nursing Assistant (CNA) from an accredited program with current state licensure or registration Demonstrated experience working in diverse healthcare environments including long-term care facilities, hospitals (including ICU or trauma centers), assisted living communities, or home care settings Familiarity with EMR/EHR systems such as PCC, Cerner, eClinicalWorks, or similar platforms for medical documentation and patient records management Knowledge of medical terminology, physiology, anatomy, and infection control practices necessary for safe patient care Experience providing specialized care such as Alzheimer's/dementia memory care, behavioral health support, pediatric or geriatrics caregiving Ability to perform physical tasks including lifting with Hoyer lifts or working with patients requiring mobility assistance Strong communication skills to effectively collaborate within multidisciplinary teams while maintaining confidentiality per HIPAA standards Join us in delivering compassionate healthcare that truly makes a difference! $22.00 - $29.00 per hour
