Job Description

Transportation CNA Palmyra , PA 17078 Quick Apply Overview Salary Range $19.00 Hourly Quick Apply Description About the Position Londonderry Village is committed to providing person-centered, individualized care and services to the elders we serve. The Transportation CNA coordinates appointments for elders and provides transportation to and from appointments in a safe and professional manner. Essential Duties and Responsibilities Schedule and coordinate medical appointments for elders. Inform elder and/or the nursing department of scheduled appointments. Transport elders to medical appointments using a fleet vehicle. Assist elder with activities of daily living while on appointments as necessary and follows elder's Care Plan. Report any fleet vehicle problems to maintenance for correction or repair. Assist elders into and out of vehicles including using the vehicle wheelchair lift and/or ramp. Lifts wheelchairs, scooters, and/or walkers into/out of vehicle as necessary. Obtain all required paperwork for the appointment from the nursing department. Pick up elders for appointments and/or discharge from the hospital. Communicate any elder issues to the nursing department for follow-up. Maintain and protect confidentiality of elder information including, but not limited to, financial and health information. Use Microsoft Office software for compiling schedules and other documents. Ensure safety of elders by observing all safety policies and practicing defensive driving. Prepare trip reports after each transport and accident/incident reports as necessary. Performs other duties as required. Qualifications Maintain a valid Pennsylvania Driver's License. Maintain PA Nursing Assistant Certifications with no annotations. Maintain First Aid and CPR certification. Have the ability to obtain a PA CDL Driver's License. Daily local travel. Must follow all universal precautions procedures, including use of personal protective equipment, due to potential for exposure to bloodborne pathogens. Share job details to
