Company overview: Lumina Care is focused on unifying, coordinating, and managing care for geriatric patients in nursing facilities and at home. We offer a range of services to improve health outcomes and quality of life for patients, including afterhours telehealth, transitional care, chronic care, remote patient monitoring, behavioral health and telehealth psychiatry.

Clinical Program Facilitator Purpose/Summary:
The Clinical Program Facilitator serves as the primary on-site representative of Lumina Care, responsible for building strong relationships with partner facilities and onboarding patients into Lumina Care's clinical programs, including CCM, BHI, and CoCM. This role plays a critical part in ensuring a positive initial experience for patients, facility staff, and providers while supporting compliance, education, and operational excellence. The Facilitator also trains staff on the use of the Emma Care platform for after-hours telehealth services.
Role & Core Responsibilities:
Patient & Facility Engagement Serve as the on-site "face of Lumina Care" at partner facilities Build and maintain positive working relationships with facility leadership, staff, and patients Conduct patient onboarding for CCM, BHI, and CoCM programs Educate patients and families on program benefits, consent, and participation requirements Ensure patient enrollment documentation is completed accurately and timely Program Implementation & Training Facilitate initial site visits for new facility implementations Train facility staff and care teams on Lumina Care workflows and expectations Provide hands-on training for the Emma Care platform, including after-hours telehealth functionality Support troubleshooting and user adoption during the onboarding phase Act as a liaison between facilities and Lumina Care clinical and operations teams Documentation & Reporting Maintain accurate records of patient onboarding activity Prepare and submit required reports, checklists, and visit summaries Utilize tablets and mobile technology to complete documentation in real time Travel & Logistics Travel to assigned facilities to conduct in-person onboarding and training Coordinate schedules and visits with internal teams and facility contacts Manage travel planning efficiently while maintaining professionalism and punctuality
Qualifications:
Required Qualifications High school diploma or equivalent required; associate or bachelor's degree preferred Experience in healthcare, care coordination, patient engagement, or clinical operations Strong interpersonal and communication skills with the ability to engage diverse populations Proficiency in Microsoft Excel and Word Comfortable using tablets, mobile applications, and web-based platforms Ability to learn and train others on new technology systems (including Emma Care) Reliable automobile with valid driver's license and insurance Ability to travel up to 50% of the time Preferred Qualifications Experience working in a Skilled Nursing Facility Experience onboarding patients or training staff Familiarity with telehealth platforms Bilingual (Spanish/English) a plus Licensed Practice Nurse, Registered Behavior Technician or Community Health Worker experience Working conditions: Consistent with the Americans with Disabilities Act (ADA), Lumina Care provides reasonable accommodations to qualified individuals with disabilities unless doing so would cause undue hardship. Compensation:
$20-$40 per hour. Compensation is determined based on market range, experience, skills, education, and qualifications.
