As a trusted caregiver, you will ensure patient comfort, safety, and dignity while collaborating with multidisciplinary teams to promote optimal health outcomes. Your flexibility and dedication will help us meet the unique needs of each facility we serve. Responsibilities Provide direct patient care including assistance with activities of daily living such as bathing, dressing, feeding, and mobility support using equipment like Hoyer lifts and other assistive devices. Monitor and record vital signs, observe patient conditions, and report changes promptly to nursing staff. Maintain accurate medical documentation within Electronic Health Record (EHR) systems such as Point Click Care, ensuring confidentiality in accordance with HIPAA regulations. Support patients with specialized needs including dementia care, Alzheimer's care, and memory care,. Implement infection control protocols and adhere to safety standards. Qualifications Valid Certified Nursing Assistant (CNA) certification required; additional certifications such as Basic Life Support (BLS) are preferred. Demonstrated one year of experience in a long-term care (nursing homes, assisted living) setting. Familiarity with medical terminology and medical documentation practices. Experience working with specialized population of geriatrics (geriatrics).
Proficiency in using EMR/EHR system:
Point Click Care. Ability to perform vital signs measurement accurately and utilize patient handling equipment like Hoyer lifts safely. Strong communication skills coupled with a compassionate caregiving approach; flexibility to travel locally between different facilities as needed.
