Job Description

Overview Join our dedicated healthcare team as a Patient Care Assistant (PCA) / Certified Nursing Assistant (CNA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our residents. In this vital role, you will provide personalized assistance to individuals in assisted living, nursing homes, or home care settings, ensuring their safety, comfort, and well-being. Your expertise in patient observation, medication administration, and supporting activities of daily living (ADLs) will help foster a nurturing environment that promotes dignity and independence. Duties Observe and monitor patients regularly, noting changes in condition and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist residents with activities of daily living such as bathing, dressing, grooming, toileting, and mobility using tools like Hoyer lifts when necessary Administer medications following prescribed protocols while maintaining strict compliance with HIPAA regulations and medical documentation standards Support residents with specialized care needs including Alzheimer's disease, dementia, memory care, hospice care, and catheter care Take vital signs such as blood pressure, pulse, temperature, and respiration to track health status effectively Assist with meal preparation and feeding to ensure proper nutrition and hydration for all residents Maintain a clean and safe environment by performing light housekeeping tasks like laundry and general cleaning Requirements Valid CNA certification or PCA Certification with current CPR certification required Prior experience working in nursing homes, assisted living facilities, or home health environments preferred Familiarity with medical terminology and documentation practices within EMR/EHR systems Experience caring for individuals with developmental disabilities or memory impairments such as Alzheimer's disease is highly desirable Ability to work with compassion and patience while supporting residents through their daily routines Knowledge of infection control procedures and resident safety protocols including hospice care considerations Strong communication skills to collaborate effectively with healthcare teams and family members Join us in providing compassionate, high-quality care that enhances the lives of those we serve.
