Responsibilities Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor patient health by taking vital signs such as blood pressure, pulse, temperature, and respiration; document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration following prescribed protocols while maintaining compliance with HIPAA regulations to ensure patient confidentiality. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques. Perform medical tasks such as catheter care, wound dressing assistance, and hospice support to meet complex health needs. Prepare nutritious meals tailored to dietary restrictions and assist with feeding when required; maintain cleanliness by performing light housekeeping tasks like laundry and cleaning to ensure a safe environment. Document all patient interactions thoroughly in medical documentation systems and communicate effectively with healthcare teams regarding patient status or changes. Requirements Proven experience working with seniors or individuals with disabilities in settings such as assisted living facilities, nursing homes, or home health environments. Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate record keeping. Certification as a CNA, HHA, PCA, or equivalent; CPR certification is required for emergency response preparedness. Ability to perform physical tasks including assisting with transfers using Hoyer lifts and working with mobility aids safely. Strong observational skills for patient monitoring and recognizing signs of distress or changes in condition. Compassionate attitude combined with excellent communication skills to build trust and rapport with clients and their families. Flexibility to work various shifts including live-in arrangements if needed; willingness to adhere to HIPAA guidelines and maintain patient confidentiality at all times.
