Job Description

Overview Join our compassionate adult family home as a dedicated Caregiver HCA/CNA, where your energy and expertise will make a meaningful difference in the lives of our residents. This vital role offers an opportunity to provide personalized, respectful, and high-quality care in a warm and supportive environment. You will be an essential part of a team committed to promoting independence, dignity, and well-being for adults with diverse needs, including those requiring memory care, behavioral health support, and complex medical assistance. Your proactive approach and genuine empathy will help create a safe, nurturing space where residents thrive. Duties Deliver direct patient care including assistance with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor residents' vital signs and observe for any changes in health status, promptly reporting findings to supervising medical staff. Administer medications accurately following prescribed schedules and protocols while maintaining detailed medical documentation within EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems. Support residents with specialized needs such as dementia care, Alzheimer's care, behavioral health management, and working with individuals with developmental disabilities. Assist in implementing individualized care plans that promote independence and enhance quality of life. Maintain a safe environment by adhering to infection control procedures, aseptic techniques, and HIPAA regulations to protect patient confidentiality. Collaborate with healthcare professionals during acute or urgent situations including emergency response procedures involving CPR or basic life support when necessary. Experience Prior experience in long-term care settings such as assisted living facilities, nursing homes, or hospice environments is highly valued. Demonstrated knowledge of medical terminology, anatomy, physiology, and patient service protocols across various healthcare settings including hospital medicine or ICU experience. Experience working with diverse populations including pediatrics, geriatrics, patients with disabilities, or those requiring sub-acute or post-acute care. Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical documentation and record keeping. Proven ability to handle complex medical procedures such as catheter care, phlebotomy, medication administration, and vital signs monitoring. Critical care experience including Level I Trauma Center or Level II Trauma Center exposure is advantageous. Strong caregiving background involving behavioral health support or working with individuals experiencing dementia or memory-related conditions.
