Job Description

Job Overview We are seeking passionate and dedicated Certified Caregivers, including Home Care Aides (HCA) and Certified Nursing Assistants (CNA), to join our team in providing exceptional in-home and community-based care. As a vital member of our caregiving team, you will deliver personalized support to individuals across various settings, including assisted living facilities, nursing homes, and private residences. Your role will focus on enhancing quality of life, promoting independence, and ensuring the safety and well-being of each patient through compassionate, professional care. Duties Provide comprehensive personal care assistance, including bathing, dressing, grooming, and mobility support for patients with diverse needs. Assist with Activities of Daily Living (ADLs) such as eating, toileting, and transferring using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, administering medications as prescribed, and documenting medical records accurately using Electronic Health Record (EHR) systems. Support patients with memory care needs, including Alzheimer's disease and dementia care, ensuring a safe environment tailored to their cognitive requirements. Maintain patient confidentiality in compliance with HIPAA regulations while managing medical documentation and records diligently. Conduct patient observation to identify changes in condition or behavior and communicate findings effectively to healthcare professionals. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment aligned with home health care regulations. Utilize EMR systems for accurate documentation of patient interactions and medical updates. Support hospice or end-of-life care by providing compassionate emotional support and physical assistance to patients and their families. Follow all safety protocols including first aid, CPR certification, basic life support training, and proper handling of medical equipment like catheters or IVs when applicable. Qualifications Valid certification as a CNA or HCA with current licensure recognized by state authorities. Proven experience working with geriatric patients or individuals with developmental disabilities; assisted living or nursing home experience is highly preferred. Strong knowledge of medical terminology, medication administration procedures, and home healthcare regulations. Experience working with individuals with dementia or Alzheimer's disease; familiarity with memory care techniques is advantageous. Ability to perform heavy lifting safely and efficiently while maintaining patient comfort; experience with patient transfer methods such as Hoyer lifts is desirable. Excellent communication skills for effective client interaction and team collaboration; ability to maintain patient confidentiality at all times. Knowledge of HIPAA compliance standards and medical documentation practices using EMR/EHR systems. Certifications in first aid, CPR, and basic life support are required; additional certifications related to hospice or palliative care are a plus.
